nvidia driver causing system freeze?

If the GLX extension is missing then the OS is breaking it.  Is he
certain that it was there originally?

That would be the best place to start whenever there's instability.
If that doesn't help, then trying a *much* newer kernel would be the
next step.  Send me an nvidia-bug-report.log if neither helps.
