[SBU Board] Draft of existing return-to-play survey

During our May 3 board meeting, I showed results from an initial survey I
had conducted with players in the SBC rookie game.

It was requested that I provide a printed copy of the existing survey.
I've created a PDF, and posted it in one of our shared folders on Google
