[SBU Board] Unit Games/Events Survey

Hi Eric!

I went through and added comments to the survey on a few things.

1) The logic isn't working as you described. I said I went to some but not
all of the events listed and it still prompted for why. (I noted in the
survey comments that I think that needs to be broken up more or have more
specific logic to get good data, as the reasons for not attending one
versus another can be very different.)
2) I have a surveymonkey account that is linked to our mailchimp account.
It's the one I used to comment. Ideally, we'd build or migrate the survey
to that username so I could send out from there. I can give you the login
if you want to use it to build the survey.
3) We do not have a paid account. When I did it before, I had a paid
account because I was using it for another effort.  To do this, we should
assume at least 2 months of paid account need, so figure $70 of cost plus
whatever prize cost you have.  We had 192 responses on the tournaments
survey last year.  FWIW, I wouldn't mind adding a few questions about
tournaments/sectionals, in part to measure versus the scoring we got last
year.
4) Procedurally, doing the survey (cost + what topics to cover + prize to
offer) is a proposal to put before the board at a meeting and get a vote,
since there's expenditure for the board and we'd end up sending out a
unit-wide communication for it.
