[SBU Board] Unit Games/Events Survey

Hi all,

I put together a survey I was hoping to work with JC to send out in the 
near future.

  A few questions:

1) Any comments/suggestions/etc on the survey? Note: there are 2 
questions that involve special logic. If you say you didn't participate 
in the Round Robin it asks for reasons why. Also if you did not 
participate in ANY of the other games (trifecta/captain's game/Round 
Robin party) it asks why.
2) What is the process for sending this out (this is 95% aimed at JC)
3) Due to wanting to handle more than 100 responses and also involving 
question logic, it requires a signing up for the monthly plan which 
would be $35. Do we already have a paid SurveyMonkey account? If not, 
what process should I go through to send this out? Mostly aimed at JC/John.
4) I noticed we entered people into a drawing for free sectional entries 
for the tournament survey. Is it okay if we offer something similar for 
this survey? 1-2 free entries to a club game reimbursed by the unit for 
2-3 winners?
