If you go the Linux route. You would need to use a combination of wine 
and ltsp/xrdp.

Wine may not support all the required functions of Medisoft then you 
would be up for paying crossover to get it working.  The amount of 
testing to get it working on wine may be huge, then add the same amount 
of testing for each version.

Medisoft might not support it.

There would be hidden traps if the application requires installation of 
mapi32.dll or outlook for email functionality... So a "Linux" install 
may still require a significant MS investment.

My thoughts only...
