Old problem revisited - X ignoring mouse clicks

A puzzle for your TG weekend.

Just to revisit some old stuff. I've encountered the same problem on
every distro and XFree86/xorg version and mozilla/Firefox and distro
I've used.

At least 4 out of 10 times when I visit a link or a button on a
browser window, the first mouse click is ignored and I have to click
the second time to activate the link or function.

I've never understood this behavior. Does anyone else?
