Combining PDF files

Does anyone know of some software which will combine
multiple pdf files into one big file and make a table of
contents, sorta like html2ps but for pdf (or postscript)
files. I want each document to be renumbered as necessary.

It would be nice if the software would allow editing of
the pdf files, too.

These file are also available in .doc format. Would it be
easier just to work with .doc format and forget pdf?
