I think RHEL ( or CentOS ) satisfies your 2, 3 and 4.

1 depends on how you define "simple" :)  I used to install servers using 
the kickstart files and just let it run. The first time it took me some 
time to figure out the disk options, but after that I could just re-use 
the files with some changes that correspond to the configuration of the 
servers.
