Just to close the loop ... I eventually solved this problem by
installing a PCI card that provides modern USB 2.0 ports instead of the
USB 1.1 ports that are on this old server. No issues with properly
detecting connected devices now. FWIW here's the card I used, seems to
work great with Linux:
