How To Create a Table of Contents in a PDF Document

Does anyone know if there is a FOSS way of adding a TOC to a PDf document? I'm 
familiar with pdftk, ps2pdf, enscript, etc. but haven't been able to figure 
out how to add a table of contents to a pdf document a la Adobe Acrobat. 
Trying to Google for an answer is like looking for a needle in a hay stack 
what with the words "table of contents" and PDF in a gazillion web pages.
