A sense key (SK) value of 4 (4h == 4 hex) indicates a hardware error.
Specifically, "HARDWARE ERROR: Indicates that the device server detected
a non-recoverable hardware failure (e.g., controller failure, device
failure, or parity error) while performing the command or during a
self test." The combination of the additional sense code (ASC) and
additional code qualifier (ACQ), which are 08/03, in this case),
translates to "logical unit communiation/CRC error" in the ultra-DMA/32
command set.
