And you're sure there isn't a firewall problem?

Can you connect to port 25 of your server IP from outside your server LAN?

What domain is it?

Does it have MX records correctly defined in DNS for that domain?

What does the qmail-smtpd logs show?

Lots of variables here but no solid data to suggest a troubleshooting
direction.
