 The hsync & vrefresh look too low to me. If they are set lower than
your hardware is actually capable of then you will not be able to use
the higher resolution settings. 

For example, my 19" KDS monitor uses a hsync range of 31.50-79.00 kHz
and a vrefresh range of 50.00-100.00 Hz
