create the template IN Kontour or Gimp. Sizing should be simple enough, 5 15/16" X 4 5/8" for the insert, and 4 3/4" X 4 3/4" for the front panel. The insert also could have lines 1/4" in on both sides for folding.
Template = outline to start from and cut/fold on.
