It was... the details escape my brain, but it was a windows box that ran a 
communications application. I recall that as part of it's monthly 
maintenance, it was to be rebooted so as to return the running application to 
a known state. For what ever reason, it failed to be rebooted at the 
scheduled time and lead to the failure... which avalanched into a complete 
crash of the airline computer systems on most of the west coast...

Amazing, trusting human lives to a windows box...
