I have a printer called brother3 with a printer type called fpml.
Printer type fpml code 59 has the following sequence:
$1b &f0S $1b *p0X0Y %"c?\tmp\letter3quote200Color.pcl" $1b &f1S
(That was taken from Jim Asman's MKPCL manual substituting the location of my ,pcl file)
I have an Output format called quote2
Quote2 'Output format options' has a 'Printer name' brother3 with Initialization print code number' 59
There are no print codes within the Output format.
It a simple output format showing client name, address, ... with entries for products purchased. 
The only processing is looking up client information and items purchased.
It's only 1 page long.  When I print the report I get <page> in the upper left hand corner of the page.  It also kicks out a 2nd page with only <page> in the upper left hand corner of the page.
